1. Acceptance of Terms

By accessing this website or using our services—including telehealth and online scheduling—you agree to abide by these Terms and Conditions. Continued use of the following updates implies acceptance of revisions.
 

2. Services Provided

Axis of Mental Health Care offers outpatient mental health services, including:

  • Individual therapy

  • Psychiatric consultation

  • Secure online scheduling

  • Telehealth appointments

All services are delivered with clinical discretion and are subject to availability.
 

3. Client Responsibilities

Clients agree to:

  • Provide accurate health and personal information

  • Use services appropriately and lawfully

  • Keep login credentials secure

  • Notify us of relevant changes (contact info, health status, etc.)
     

4. Privacy and Confidentiality

We uphold the highest privacy standards:

  • Information is never sold or shared without written consent

  • No data is used for marketing purposes

  • Our platform is HIPAA-compliant

  • Telehealth and scheduling systems are secure and encrypted

5. Limitations of Telehealth

Telehealth is valuable but not suitable for all situations. You agree to:

  • Participate in a private, distraction-free space

  • Use a reliable internet connection

  • Acknowledge that emergency care may require an alternate referral
     

6. Cancellation and No-Show Policy

  • Appointments must be cancelled at least 24 hours in advance

  • Late cancellations or missed appointments may incur fees

  • Excessive no-shows may restrict future scheduling access
     

7. Intellectual Property

All site content—including branding, text, and images—is property of Axis of Mental Health Care and may not be reused or distributed without prior written consent.
 

8. Liability Disclaimer

Axis of Mental Health Care is not liable for:

  • Platform outages or third-party scheduling issues

  • Unauthorized access due to password sharing

  • Decisions made based on website content
     

9. Governing Law

These Terms are governed by the laws of the State of Texas and are subject to the jurisdiction of Texas courts.

Client Rights & Access to Mental Health Records

(Texas Health & Safety Code §181.105)

We are committed to respecting your privacy, dignity, and rights as a mental health client. In accordance with House Bill 4224 (89th Regular Session), effective September 1, 2025, we provide the following information to help you access your mental health records and understand your options if you have concerns.

🗂️ How to Request Your Mental Health Records

You have the right to request access to your mental health and treatment records maintained by our organization, as permitted by Texas and federal law.

To request your records:

  1. Submit a written request to our office (email, mail, or in person).

  2. Please include:

    • Your full legal name

    • Date of birth

    • The type of records you are requesting (e.g., treatment summaries, assessments, discharge records)

    • How you would like to receive the records (secure email, mail, or in-person pickup)

    • A valid form of identification

  3. Requests are processed in accordance with applicable privacy laws and timelines.

If you have questions about your rights related to mental health records or health information privacy, you may contact the Council directly.

Please submit questions through the Council’s Contact Us webpage to ensure your inquiry is directed to the appropriate staff.

⚖️ File a Consumer Complaint

If you believe your rights related to mental health records or privacy have been violated, you may file a complaint with the Office of the Attorney General of Texas – Consumer Protection Division.
